Lung Abscess

I am 46 year old lifelong smoker until 1 1/2 months ago. I started coughing up blood (not streaks or spots) almost 3 weeks ago, I didn't have a bad cough in fact when I first saw blood I thought it must be some healing thing from quitting smoking. I cough only a couple of times a day but there is always blood when I bring it up from lungs. The X-ray showed an abscess (I have had for at least 10 years) and several calcified spots. CT Scan showed several calcified granulomatous and noncalcified nodules in lower left lobe and the lingula. I also have cysts in my spleen and a 23 mm low attenuating mass on my adrenal gland. The doctor says that the abscess is spewing bacteria all over my body. Up until 1 1/2 weeks ago I have been very hyper, not eating and not sleeping. Then for 1 1/2 weeks I was very tired now I am hyper and on edge again and not sleeping. The doctor has me on 250mg(x3) Metronidazole until I can cough up enough they can test. I have been trying for 2 weeks to get enough for a sample but I can only cough up pea size always with red blood. I am not running a fever but I never do even for pneumonia and serious kidney infections. My white count is elevated but not too high. I also have cysts on my ovaries, ears and the doctor feels like there are probably many more in my body. Should I be getting a much more agressive treatment, should I have a biopsy, or should I just wait and see if the medicine works?

I forgot to add. My Ferritin (iron) count was at 3 and has been ridiculously low for a very long time.
