diagnosis of sarcoidosis

I recently diagnosed sarcoidosis I am still not convinced I have disease. It all started severe pain in my abdomen, shortness of breath weight loss.y did a sonogram of my abdomen found several swollen lymph nodes around pancrease liver. n did a biopsy of lymph nodes it came back as noncaseating granulomas. During time my blood work showed high AST SGOT sed rate.I am now on predisone 40 mg I still have pain in my abdomen although recent ct scan shows nothing at time. Could be sarcoidosis or something else?

It could be sarcoidosis, although this would be a very atypical presentation for it. Sarcoidosis is a systemic disease of unknown cause which can present in a multiple of different organs and locations. Although it usually presents in the lung. Other common places are the eyes, joints and skin. The liver can become affected as well, along with other organs. The diagnosis is one of exclusion, exluding other things such as infections from tuberculosis (tuberculosis causes caseating granulomas) and fungal infections. Rarely other infections can cause this. Rarely other diseases can be confused as well. It is of course impossible to give a diagnosis without a thorough review of everything available. One would feel somewhat assured though if the repeat scan after treatment with steroids showed resolution of the lymphnodes, but this does need to be followed closely. The abdominal pain may be unrelated and I would let your physician know that the pain is still there and of your concerns.
