I was (60 yrs old) diagnosed DVT on 10th Oct 08. Given 5 Clexane injection and then 3 mg warf. Then dose was increased to 5 mg and then 7 mg. after t7 mg INR was 2.76. Then 7 mg was continue for 10 days/ latest INR is 8. Is it too large value? What to do next.. what should be the dose now?

Typically an INR should be 2 to 3 for DVT therapeutic purposes. 8 is too high. The dosage needs to be titrated down to achieve those values. Can't say what the dosage should be because it is determined on your personal values. Interact with your physician.
